Secondary (High School)

If your official transcript does not confirm graduation, we will require an official diploma or statement confirming graduation and the graduation date.

           Official Transcript confirming completion of secondary education or graduation from:

Alberta:  Electronic transcript sent directly to FACS by MyPass

British Columbia:  Electronic transcript sent directly to FACS by StudentTranscripts Service

Saskatchewan:  Electronic transcript sent directly to FACS by MyCreds

Yukon:  Electronic transcript sent directly to FACS by StudentTranscripts Service

The following provincial documents should be sent directly to FACS by postal mail or email:

Newfoundland, Northwest Territories, Nunavut, or Québec:  Official transcript issued by the Ministry or Department of Education.

Manitoba: Official transcript issued by the Department of Education.

Nova Scotia or Ontario:  Official provincial transcript issued by the school

New Brunswick, Prince Edward Island, or Québec (for DEC):  Official transcript issued by school.

Post-Secondary (University):

           Official transcript or grade report sent directly to FACS from the issuing institution. If your institution participates in the digital delivery of transcripts from MyCreds, please arrange for delivery to FACS through this platform.   ________________________________________________________________________________________________________________________________

General Information for All Applicants

This is important! To save time for both you and us, please keep the following in mind:

Secondary Credentials (High School) are only required if one of these conditions exists:

  • Your potential employer or school needs to see your high school studies.
  • Your high school education included Advanced Level Studies.

FACS only uses official educational documents when preparing your evaluation.  Do not send photocopies or scans of your educational documents.

Documents not issued in English must have official translations completed and sent to us.

Do not have your school send your documents until AFTER applying for the evaluation.

If your educational institution requires a written document request, please use our Academic Records Request Form.  Fill out your details, and a PDF will be emailed  to you. You may then submit this PDF to your educational institution.  They will complete their portion of the form and submit it to FACS with your official records.

Secondary (High School)

The original* hard copy of the following:

Diploma de Ensino Medio /Certificado de Conclusao de Ensino Medio / Diploma de Tecnico de Nivel Medio / Diploma de Ensino Medio com Habilitacao or historica escolar confirming graduation from Ensino Medio / Segundo Grau.  If this document does not clearly state that you have graduated or completed the high school program with a graduation date, you must additionally provide a graduation certificate.

Post-Secondary (University)

The original* hard copy of the following:

  • Histórico Escolar (Scholastic History)
  • A photocopy or scan (from you) of the Diploma for the Titulo/Grau (Title/Degree) or Certificado (Certificate) for “Lato Sensu” programs

*An original document is the first issue document given to the student from the institution, typically at the completion of studies or at the time the degree/diploma is awarded. These documents are usually printed on security paper, have a raised seal, are stamped with ink, or include a signature of authorized personnel.
