Post Secondary (University)-Required Documents
FACS Requires
- Degree Certificate
- Transcript/grade report
- Curriculum/study plan (only if available, hours of study or credits for
each course if not included on the transcript)
Type of Documents Accepted
FACS will accept hard-copy transcripts in a university-sealed envelope mailed to our office by the applicant or the institution. If you are sending the document, the envelope must remain sealed, or the document will not be accepted. We will also accept an e-transcript via email directly from the issuing institution. Please have your institution send official e-transcripts to [email protected]. Note that this email address is for institutional use only.
Please use our Academic Records Request Form if you need to request your academic records from your institution or school. Fill out your details, and a PDF will be emailed to you immediately.
If you are unable to provide an official transcript in a university-sealed envelope, you may have your degree and transcripts verified through

Secondary (High School)-Required Documents (Optional – only required if you need High School included in your report)

- Grade Report of Secondary School. We require original documents or official documents is a sealed envelope.
