We need your marksheets, degree certificates, & professional qualifications.
Please provide an AFFIDAVIT OF NAME CHANGE if your name differs from your official documents.

APPLY

Click on the Applications link in the menu and select the appropriate form, depending on your needs. Fill in the form. At the end of the application form, you will be able to pay the evaluation fee online. When you’ve completed the form, and click “Submit,” you will receive a confirmation, a summary of your request, and a FACS Application ID number. You may print this page for your reference, although a confirmation email is already on the way.

SEND DOCUMENTS

After you’ve hit “Submit” at the end of the form, and received your confirmation, you’ll need to send us your education documents via postal mail. Please contact your institution(s) to have an official transcript and certificate of degree sent to our office. ACADEMIC RECORDS REQUEST LINK HERE. Please ask them to include the FACS Application ID number, if possible.

Once you’ve completed Steps 1 and 2, relax, and let us prepare your advisory evaluation. If we have any questions, or are missing documents, we will contact you via email (please add [email protected] to your contacts so our mail goes to your inbox). When the evaluation is complete, we will mail the original evaluation to the intended recipient, and a copy is emailed to you. Please feel free to contact us if you have any questions. Enter all education on your application that you would like to have evaluated on your application. BE COMPLETE AND ACCURATE – only education included on your application form will be evaluated. Guam Applicants: If you are including courses that you have not yet completed list each subject separately in the anticipated credits portion of the application.

Acceptable Marksheets

OPTION 1. University/College-attested individual marksheets, issued in an ENVELOPE STAMPED AND SEALED BY THE CONTROLLER OF EXAMINATIONS, REGISTRAR OR ASSISTANT REGISTRAR OF THE INSTITUTION YOU ATTENDED or, emailed to FACS directly from the University. ([email protected])

OPTION 2. Your original, first issue marksheets posted to us through courier to:

UNACCEPTABLE MARKSHEETS

Photocopies and/or scans of your marksheets sent by the applicant.

ACCEPTABLE DEGREE CERTFICATES

Photocopy of your official degree certificate(s) or provisional degree certificate issued by the university or autonomous college. This should be mailed to our office. If your university can include this certificate with your mark sheets, you do not need to send it. If they do not, you need to email it to [email protected]

UNACCEPTABLE DEGREE CERTIFICATES

A degree certificate issued by the affiliated college rather than the university. (Example: An affiliated college of University of Madras cannot issue a degree certificate. We require the degree certificate from the University of Madras)
