  • Titulo (Title)
  • Professional Cédula Card
  • Grade report / academic record
  • Curriculum/study plan (only if credits for each course are not on the transcript)

FACS requires a photocopy or scan of the front and back of the document for the Professional Cédula Card and the Curriculum/study plan.

FACS requires the original Titulo (Title) and the original Grade Report

An original document is the first issue document given to the student from the institution typically at the completion of the studies or at the time the degree is awarded. The official documents are usually printed on security paper and have a raised seal and/or fresh ink stamp.
